AFFIDAVIT OF TWO DIS-INTERESTED PERSONS

We, MAIDA SAGUID BANTASAN, married, of legal age, Filipino Citizen, with residence and postal
address at Kennon Road, Camp 3, Tuba, Benguet, and LETICIA D. ARONZA, married, of legal age, Filipino
Citizen, with residence and postal address at Kennon Road, Camp 3, Tuba, Benguet, after having been duly
sworn to in accordance with law hereby depose and state under oath, that:

1. We personally know SUPING BAYENG MATIAS, now SUPING MATIAS ARISTON, the daughter
of SERAFINA BAYENG and SUNNY MATIAS;

2. Her mother SERAFINA BAYENG MATIAS is a member of the Blessed Association of Retired
Persons Foundation, Inc. (BARP) and her beneficiary is SUPING BAYENG MATIAS;

3. Serafina Matias died on March 10, 2023 at Tuba, Benguet and days after her burial, her children started
to process all the documentations regarding the BARP benefits, however, it was found out that what was
written as the name of the beneficiary is “JOSEPHINE ARISTON” instead of “SUPING
ARISTON”;

4. The correct name of the beneficiary of the late Serafina Matias is “SUPING ARISTON” which is her
registered name and which name she’s been using since her younger days up to the present. However,
just like other people, Suping has an alias name or nick name which is “JOSEPHINE ARISTON”.
This name “JOSEPHINE”, is much more popular to the community and to us instead of her registered
name “SUPING”;

5. We are used to call Suping in her alias name or nickname “JOSEPHINE”. Thus, maybe when her
mother applied for BARP membership, the mother or the personnel who entered the details
inadvertently entered the name “JOSEPHINE ARISTON” instead of the registered name “SUPING”;

6. With the foregoing circumstances, the name “SUPING BAYENG MATIAS now SUPING MATIAS
ARISTON” and “JOSEPHINE ARISTON”, pertains to one and the same person and the biological
daughter of the late Serafina Bayeng Matias;

7. We are not in any way related to the above-mentioned person neither by consanguinity nor affinity;

8. We know the above facts and circumstances as we are long time friends of the aforementioned person;

9. We are executing this affidavit to attest to the truth of our foregoing statements and for all legal intents
and purposes it may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 27th day of March, 2023 at Baguio City,
