Filipino, of a legal age, Resident of Kalanawan, Manolo Fortich Bukidnon hereinafter referred as the DONOR.

the office address at Puntian Sumilao Bukidnon here called the DONEE

WITHNESSETH:

That for consideration of the desire and support to contribute a share for the cause of education for the
Filipino learners through adopt –A- school program of the Department of Education, the DONOR has freely and
voluntarily given, transferred and covered by way of donation to the DONEE, its successors and assign free and
clear of any and all liens and encumbrances whatsoever all its right, interest and title on.
