    Temperature Record Sheet


    1. Parents/Guardians should take their children’s temperature before going to school. If the
    temperature is over 99.0F or 37.2C, students should not attend school and should consult a
    doctor promptly.

    2. Parents/Guardians should keep a record of their children’s temperature and sign on the record
    sheet daily. Students will be required to present their daily temperature records to the
    invigilating teachers during examination period.

    3. Please also take the temperature reading on school holidays and make adequate copies for your
    needs.
