from the date issued (above) unless you have received correspondence stating otherwise.
YOU WILL NOT RECEIVE YOUR NEW CARD IF YOU DO NOT LIVE AT THE ADDRESS LISTED BELOW ON THIS
DOCUMENT. THE U.S. POST OFFICE CANNOT FORWARD DRIVER’S LICENSE OR ID CARDS.
ILLINOIS LAW REQUIRES YOU TO NOTIFY THE SECRETARY OF STATE’S OFFICE OF AN ADDRESS CHANGE.
To check the delivery status of your card, visit www.cyberdriveillinois.com and search “DL Status” or call
217-782-7044.
Keep this temporary driver’s license until your permanent card arrives in the mail. The temporary driver’s license
is not valid for identification unless presented with your current driver’s license.
Please do not visit a Secretary of State facility to request another temporary or duplicate
driver’s license/identification card. If you need to make changes, please wait until you receive
your new driver’s license/identification card to visit a Secretary of State facility.
