This form is to be completed by the individual requesting ACCESS to the Oyu Tolgoi Underground Mine Project site. This form must be
approved by the Constructi on Site Manager (or nominated delegate) prior to any travel arrangements being made.
Approval of this form does not override all other mandatory internal/external approvals, visas and alike to travel to
Mongolia. Please return this completed form to OTTravel@jacobs.com prior to any travel arrangements being made.
Incomplete forms will not be processed. Refer to the Travel Procedure for further information.

SITE ENTRY REQUIREMENTS
OT VISITOR SHORT TERM ACCESS REGISTRATION FORMS:
• Completed Visitor Access form must be returned to OTTravel@jacobs.com no later than 7 days prior to the arrival to site to ensure
adequate processing time. Incomplete forms cannot be processed.
PERSONAL PROTECTIVE EQUIPMENT (PPE):
All Visitors MUST provide and wear the site minimum PPE:
• Steel Capped Lace-up Boots
• Long Trousers or Jeans (Hi-Vis Underground).
• Long sleeved collared shirt (Hi-Vis).
• Safety Helmet, Safety Glasses, Gloves and any other specific PPE will be provided at site.
EMERGENCY PROCEDURE:
• A Visitor Induction will be provided upon arrival to site. Your site escort will be available to answer any questions you may have
whilst on site.
SITE ENTRY REQUIREMENTS ACKNOWLEDGEMENT
It is the responsibility of the Visitor to ensure they have read and understand the above site entry requirements. Failure to adhere to these
compulsory requirements will result in the Visitor being removed from site.
