Step 1: Get/Download the ODK Collect app

 The easiest way to get the ODK Collect app is to download it from the Google
Play Store on your mobile phone or tablet.
 After Installing click on the app icon to open it.
 You will see the Main Menu Screen (see picture and description of each button
below)

Main Menu

Fill Blank Form

Lists available blank forms and lets you select a form to begin filling out.
Edit Saved Form

Lists completed and saved forms and lets you select a form to edit.
Send Finalized Form

Lists finalized but unsent forms and lets you select forms to send to the
server.
View Sent Form

Lists forms that have been sent, even if they were deleted.
Get Blank form

Lists blank forms available on the server and lets you download them.
Delete Saved Form

Lists all the Saved and Blank Forms and lets you delete them.

Step 2: Configure the ODK Collect app


1. Click the Action Button ( ⋮ ), and select  Configure via QR code

2. Position the attached QR code in the centre of the camera field, under the
red line. When the camera focuses on the code, it will beep and scan the
code.
3. ODK Collect will apply the settings from the code
 Select  Send Finalized Form  to upload your completed survey.
STEP 4: Fill Blank Form

 Click on Fill Blank Form

Step 5: Select a Form


 Click on OPHID Staff Trainings Tracker and follow the instructions

below ↓.

Step 6: Select your name

 Type your name in the type bar when your name appears select it.
 Click on Next> .
Step 7: Confirm details

 Click on “Yes” and then Next> . OR


 If the information provided is incorrect click on “No” and then Next> .
Then follow instructions given.

Step 8: Select the relevant Staff Training

 Click on the relevant training and then Next> .

Step 9: Complete module section


 Complete the required section as indicated and then Next> .
Step 10: Complete module section

 Complete the required section as indicated and then Next> .

Step 11: Complete module section


 Complete the required section as indicated and then Next> .

Step 12: Complete module section

 Complete the required section as indicated and then Next> .

Step 13: Complete module section


 Complete the required section as indicated and then Next> .

Step 14: Enter Certificate Issue Date

 Click on Select date and enter the date of completion, then Next> .

Step 15: Send your Certificate


 Copy email address and send your Course/Training Certificate.
 Click on Next> .

Step 16: End of Form

 Click on Save Form and Exit .
