 BASKETBALL WAIVER

By my signature below, I acknowledge that I am aware of, appreciate the character of, and voluntarily
assume the risks involved in participating in all activities associated with basketball. By my signature
below, on behalf of myself, my heirs, next of kin, successors in interest, assigns, personal
representatives, and agents, I hereby:

1. Assumption of Risk. I understand that participating in the Activity entails inherent risks of physical
injury, including, but not limited to, the risks described in the Activity Detail Form on the reverse side of
this Release Agreement. I have been given the chance to ask questions concerning the Activity Detail
Form, and all such questions have been answered to my satisfaction. Having read this form, I am fully
aware of the risks and hazards associated with the Activity, and hereby elect to voluntarily participate in
the Activity. I voluntarily assume full responsibility for any risks of loss, property damage or personal
injury, including death, that may be sustained by me as a result of participating in the Activity.

2. Consent to receive any medical treatment deemed advisable in the event of injury, accident or illness
during these activities.

I HAVE READ THIS ASSUMPTION OF RISK, WAIVER OF LIABILITY AND RELEASE AGREEMENT. I CONSENT
TO MEDICAL TREATMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP
SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY
INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A
COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY
