INFORMATION

The undersigned Assistant City Prosecutor, upon sworn complaint originally filed by the
offended party, accuses _______________ of the crime of Unintentional Abortion, committed as
follows:

and there, willfully, unlawfully, and feloniously, while having a quarrel with
