Proposal

For

Medical contract
Between
LEKKI GRANDVIEW HOTEL
And
OH PRIME HOSPITAL
This medical contract is made this day between LEKKI GRANDVIEW HOTEL, a company incorporated
under the laws of Nigeria and having its office at 132, Nnobi lane beside Prince Ebeano (hereafter
referred to as the

"CLIENT", which expression shall where the context so admits include its

successors-in-title and assigns) of the one part

AND

OH Prime Hospital, 13A JohnVic Igboanugbo Street, Lekki Phase 1, Lagos

(hereafter to as the "RETAINER", which expression shall where the context so

admits include its successors-in-title and assigns) of the other part

WHEREAS

The CLIENT is desirous of retaining the services of the RETAINER for the

purpose of rendering General Medical Services to the CLIENT and the

RETAINER has agreed with the CLIENT to render all necessary services to the

CLIENT upon the terms and conditions contained here under:

ELIGIBILITY

All referred staff of the CLIENT are eligible to receive General medical services

from the RETAINER.

WARANTEES

The RETAINER warrants that all personnel shall possess requisite qualification

and sufficient experience to ensure that the medical service rendered accord

with required standards and professional codes.

HOURS OF OPERATION

The RETAINER'S services are available to the CLIENT 24 hours daily where the

CLIENT'S staff requires medical services. The RETAINER would require advance

notice in such cases.


ATTENDANCE

● Only the CLIENT'S staff and enrollees upon agreement are qualified to

attend.

● They shall provide the agreed documents before the screening begins
● The staff is to present a valid ID card or a medical referral or an email could

be sent from the CLIENT on each visit out of which the original will be

forwarded with the monthly bill to the CLIENT; duplicate will remain with the

RETAINER

SCREENING

The RETAINER shall have access to the following services for all referred staff

of the CLIENT ● Outpatient care.

● Inpatient care.

● Pre-employment screening.

● Annual staff check.

● Emergency Services

INVOICE AND PAYMENT

The CLIENT shall pay a security deposit of 1,000,000Naira (Negotiable), out

of which the cost of running the test on the referred staff is drawn out. The

RETAINER shall invoice the CLIENT once a month for the service charge and

total amount payable for cases handled in situations where the security

deposit has been exhausted.

Each invoice shall be accompanied with a copy of the attendance slip and shall

be paid within 30 days of the subsequent month of service rendered.

TERMINATION

The contract agreement can be terminated by either party with 30 days

written notice, citing the reasons for the desired termination of agreement.
