JOINT AFFIDAVIT

 
We, JULITO C. PETALCORIN and ERLINDA T. PETALCORIN,
both of legal age, married to each other and residing at Purok 10, Biao Tienda
Ula, Tugbok District, Davao City after having been sworn to in accordance with
law hereby depose and say:

1. That we contracted marriage on 21 September 2001 at the Immaculate

Conception Church, Mintal Davao City as evidenced by the attached
Certificate of Marriage, made an integral part hereof as ANNEX “A”;

2. That we are the biological parents of Earl John T. Petalcorin who was
born on 06 November 2000 in Davao City as evidenced by the attached
copy of the child’s Birth Certificate, made an integral part hereof as
ANNEX “B”;

3. That we hereby acknowledge our filiation to our child Earl John T.

Petalcorin.

4. That we are executing this Joint Affidavit to attest to the truthfulness and
veracity of the foregoing facts, and for the purpose of complying with the
requirements in applying for the Educational Assistance Loan Program
(EALP) Application, and for whatever purpose it may serve.
