This is your opportunity to have a voice. Often in group projects, some

members do not ‘pull their weight’ yet expect to get the same grade as
everyone else in the group. This evaluation is to be ‘anonymous’, i.e. for
your Instructor’s eyes only.

Please list all the names of your team members. Be sure to put *your name
in the top space. You must also evaluate yourself; then rate each group
member from 0 -5.

N.B. 0 means the person did nothing, while 5 indicates that you considered his/her
participation and performance completely satisfactory.
