    1. Copy of site setting out plan, indicating the area to be demolished (soft copy as per Circular no. 400)
    2. Copy of engineering survey for seismic or vibration test (only in case of controlled explosion).
    3. NOC from relevant Authorities (for Mechanical methond-DEWA, Telecom Service Provider) (for Controlled Exlosions-DEWA,
    Telecom Service Povider, Civil Defense, Police, RTA)
    4. Layout of fence / hoarding (soft copy as per Circular no. 400)
    5. Method statement, specific HSE & emergency plan along with the risk assessments.
    6. A study of the impact of demolition on the surrounding neighboring buildings to be submitted
    7. Photograph of the site

    Notes:
    • In case the customer did not start within 3 months of issuing the permit, he has to re-apply for new permit.
    • All NOC(s) must be valid during the time of demolition.

    • I hereby certify that the information, data and documents provided with this application are true, genuine and correct to the
    best of my knowledge. In case of default/failure to comply with any of the aforesaid conditions or the documents submitted by us
    is found false/forged, our application will be rejected forfeiting any applicable fees paid.
    • I hereby confirm that this project is a complete building demolition (in case of partial demolition before building
    completion certificate is issued, consultant has to submit an application of Building-Design Revision, or in case of partial demolition
    after building completion certificate is issued, it has to be submitted through Building Modification project.
