MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement executed by and between

The LALA PROPER INTEGRATED SCHOOL, a public secondary institution, with the office
address of Lala Proper, Lala, Lanao del Norte, represented herein by its School Head,
MARILOU C. APAT, Ed.D., herein after referred to as “DONEE”;
-and-
The BARANGAY LALA PROPER of Lala, Lanao del Norte a Local Government Unit
represented herein by its BARANGAY CAPTAIN, LUCILO D. OMANDAM herein referred as
“STAKEHOLDERS”.

WITNESSETH: That,

WHEREAS, the DONEE, aims to implement and strengthen the Gulayan sa Tahanan,
promote and enhance vegetable production in the home, increase KSA (Knowledge, Skills
and Attitude) among the learners on the importance of backyard/home gardening, and
educate parents how to improve nutritional status of their children;

WHEREAS, the DONEE, acknowledges the need for partners in the implementation of
Gulayan sa Tahanan Project of Lala Proper Integrated School, Lala Proper, Lala, Lanao del
Norte;
WHEREAS, the STAKEHOLDERS, will voluntarily provide assistance and other resources
to address the need of the school for the implementation of Gulayan sa Tahanan Project.
WHEREAS, the seedlings, shall be given buy the DONEE to ensure the availability of
resources in all homes under the STAKEHOLDERS and not to burden them with the
seedlings expenses;
NOW, THEREFORE, for and in consideration of above premises and mutual covenants,
agreements and stipulations hereinafter set forth, the parties hereby agree jointly to be
partners on the implementation of Gulayan sa Tahanan Project with the following
stipulations:

I. ROLES AND RESPINSIBILITIES OF THE PARTIES

1. The DONEE shall:

1.1 Conduct orientation to the STAKEHOLDERS and its constituents about the
implementation of the project, Gulayan sa Tahanan Project;

1.2 Provide seedlings to STAKEHOLDERS to ensure that everyone gets to

participate
on the project;
1.3 Closely monitor the progress of the gulayan until harvest.

2. The STAKEHOLDERS shall:

2.1 Help provide seedlings to the families under their barangay’s premises,
2.2 Assists the DONEE’s needs in terms of the profile and/or scheduling of visits;
2.3 Participates on the project monitoring its constituents and provides the require
assistance for the purpose.

II. EFFECTIVITY OF AGREEMENT

This Agreement shall take effect upon the release of seedlings to the stakeholders
for the project implementation and shall be in effect until harvest and is automatically
extended if circumstances occur may be.

IN WITNESS WHEREOF, the parties hereunto have affixed their respective signatures this

WHEREAS, the DONEE, acknowledges the need for partners in the implementation of
Gulayan sa Tahanan Project of Lala Proper Integrated School, Lala Proper, Lala, Lanao del
Norte;
WHEREAS, the STAKEHOLDERS, will voluntarily provide assistance and other resources
to address the need of the school for the implementation of Gulayan sa Tahanan Project.
WHEREAS, the seedlings, shall be given buy the DONEE to ensure the availability of
resources in all homes under the STAKEHOLDERS and not to burden them with the
seedlings expenses;
NOW, THEREFORE, for and in consideration of above premises and mutual covenants,
agreements and stipulations hereinafter set forth, the parties hereby agree jointly to be
partners on the implementation of Gulayan sa Tahanan Project with the following
stipulations:

I. ROLES AND RESPINSIBILITIES OF THE PARTIES

1. The DONEE shall:

1.1 Conduct orientation to the STAKEHOLDERS and its constituents about the
implementation of the project, Gulayan sa Tahanan Project;

1.2 Provide seedlings to STAKEHOLDERS to ensure that everyone gets to

participate
on the project;
1.3 Closely monitor the progress of the gulayan until harvest.

2. The STAKEHOLDERS shall:

2.1 Help provide seedlings to the families under their barangay’s premises,
2.2 Assists the DONEE’s needs in terms of the profile and/or scheduling of visits;
2.3 Participates on the project monitoring its constituents and provides the require
assistance for the purpose.

II. EFFECTIVITY OF AGREEMENT

This Agreement shall take effect upon the release of seedlings to the stakeholders
for the project implementation and shall be in effect until harvest and is automatically
extended if circumstances occur may be.

IN WITNESS WHEREOF, the parties hereunto have affixed their respective signatures this

The BARANGAY CAMALAN of Lala, Lanao del Norte a Local Government Unit represented
herein by its BARANGAY CAPTAIN, ALEX MALINAO herein referred as
“STAKEHOLDERS”.

WITNESSETH: That,

WHEREAS, the DONEE, aims to implement and strengthen the Gulayan sa Tahanan,
promote and enhance vegetable production in the home, increase KSA (Knowledge, Skills
and Attitude) among the learners on the importance of backyard/home gardening, and
educate parents how to improve nutritional status of their children;
