1.0 PRELIMINARIES

1.1 To provide all related insurance cover note (CAR/Workmen LS

Compensation & Public Liability) prior to the issuance of work
permit, any protection work, double handling & site clearing during
work and after work completion

1.2 Mobilisation & demobilisation of all machineries, equipments & LS

labors (rate inclusive of internal mobilisation)

2.0 TRAFFIC MANAGEMENT PLAN

2.1 To provide traffic management plan at site including the LS

appointment of a full time traffic controller , temporary traffic
furnitures such as cones, traffic sign, flagman and other associated
equipment as per the latest Malaysia Highway Authority (MHA)
Traffic Management Plan Guidelines.

(Traffic management plan drawing with TMO endorsement shall be

submitted and approved by MHA & LEKAS prior to the issuance of
work permit)

3.0 PAVEMENT REHABILITATION WORKS

3.1 To supply labours, machineries, tools, materials and equipments for

conducting pavement rehabilitation works at the affected locations

a) Mill 50mm thick in single pass of the existing pavement sq.m 182,000
including clean and remove all debris to the contractor's own
dump site

b) Apply bituminous tack coat (RS 1 K) over the milled surface sq.m 182,000
area prior to receiving the Asphaltic Concrete Wearing Coarse
(ACWC 20)

c) Lay and compact 50 mm thick ACWC 20 including applying sq.m 182,000

temporary road marking on the new surface

3.0 LABORATORY PREMIX TESTING

3.1 To submit indepencdence laboratory premix testing report for: LS

a) Coring Test
b) Determination of bitumen content and grading test
c) Marshall test (flow and stability)
d) Determination of core density and compaction test
