Three duly accomplished Personal Data Sheet (PDS) with

attached Work Experience Sheet (WES)
Two authenticated copy of Eligibility (Certificate of board rating)
with official receipt
One authenticated copy of TOR
One authenticated copy of Certification / Diploma
One original PSA Birth Certificate with official receipt
One original PSA Marriage Certificate ( if applicable )
One original PSA Birth Certificate of child ( if applicable )
One original copy of valid NBI clearance
One original copy of valid Police clearance
Two certified true copy of latest IPCR rating and service record
for former / active government employees .
If terminated , submit certified true copy of decision / resolution
coming from previous agency
One certified true copy of appointment ( CSC form 33b/ 33a ) for
former / active government employees from previous agency
One NCIP or NCMF Certificate of Confirmation ( COC ) for
applicants which do not meet the height requirement
One authenticated / certified true copies of training certificate
