film making project of MAPEH (Health) with the lesson Stress Management (Eustress, Distress

and Coping Up with Stress) at Brgy. San Juan, Palompon, Leyte, on April 5, 2023.

I have considered the benefits that my son/daughter will derive from his/her participation in this

activity and will not hold the school responsible for any untoward incident that may happen

beyond their control, provided, due care and precautions will be observed to ensure the comfort

and safety of my son/daughter.
