I, DIANADEE D. MAOMAY, of legal age, Filipino, married, and resident of Poster
Compound Prenza 1, Marilao, Bulacan after having been sworn in accordance with the
law hereby depose and state that;

1. That I personally known ISAGANI PONTANARES MAOMAY;

2. I am the wife of ISAGANI PONTANARES MAOMAY, 64 years of age, and

residing at Poster Compound Prenza 1, Marilao, Bulacan;

3. I have known him to have been baptized in Balatan Church San Isidro Labrador on
December 14, 1960 with godparents being Crising Ramos and Dionesio Maomay;

4. For records purposes, I hereby state that the name ISAGANI PONTANARES
MAOMAY is his real and complete name and his other personal circumstances are as
follows:

5. This Affidavit is being executed to attest to the truth of the foregoing facts and to
establish the identity of ISAGANI PONTANARES MAOMAY being baptized in the
