1. Submitting herewith the Monthly Accomplishment/Activity Report of this station
for the month of APRIL 2019, to wit:

I. GENERAL STATEMENT

The Municipality of Lal-lo is considered the 1 st class municipality

consisting of 35 barangays. Still growing in number, Lal-lo has many
business establishments on different assemblies and still a developing
municipality. Lal-lo Fire Station as part of the Local Government Unit
(LGU) is in close coordination with the Lal-lo Town Mayor Hon. Florante C
Pascual and sees to it that the implementation of RA NO. 9514 otherwise
known as the “Fire Code of the Philippines of 2008” is properly and strictly
implemented to ensure public safety and promote economic development
through the prevention and suppression of destructive fires.

IV. PLANS, PROGRAMS AND PROJECTS

Continuous working on the extension of the fire station.

B. PROGRAMS
To join the LGU programs in all barangay to increase the fire code
fee collection.

To continue upgrading fire volunteer and first responder in our
AOR.

V. MAJOR ACTIVITIES

CONSOLIDATED MONTHLY ACCOMPLISHEMENT/ACTIVITY REPORT

a. 03 April 2019- SINSP JAY-R P GUILLANO, BFP, SFO2 Rizal Protacio C

Jacob and FO1 Marc Kevin Colosaga, went to different school around AOR to
conduct School Inspection being constructed/ completed and those already
operational under DepEd-DPWH project in preparation for the forthcoming
school year (2020).

 b. 11 1500H April 2019- 11 1700H April 2019- Duty Personnel of this station
under the supervision of SINSP JAY-R P GUILLANO, BFP, went to
Development Bank of the Philippines Lal-lo Branch located at Magapit, Lal-lo
to conduct Fire and Earthquake Drill for the compliance and to ensure Bank
assets and its people.

c. 16 April 2019- BFP Lal-lo Personnel headed by SINSP JAY-R P GUILLANO,

BFP, coordinate with different agencies regarding the activities for the
upcoming Semana Santa.

d. 17 April 2019- 21 April 2019- Personnel of this station was place in Red Alert
Status. SINSP JAY-R P GUILLANO, BFP Municipal Fire Marshal conducted
accounting of personnel and briefing before the deployment to respective
posts (Magapit and Centro) within AOR for the Oplan SUMVAC Lakbay
Alalay 2019. Personnel are duty to guide and assist the motorist and respond
for any possible emergencies.

e. 27 April 2019- BFP Lal-lo Personnel headed by SINSP JAY-R P GUILLANO,

BFP, went to barangay Bagumbayan to have House to House Survey for
Oplan Ligtas na Pamayanan.

f. April 2019- Duty Personnel of this station went to Rancho, Baranagay San
Mariano to deliver water and Watering of Plants at LGU Park and Lal-lo
National High School.

g. Strict implementation of the provisions RA 9514/ Fire Code of the Philippines

by continuous inspection of business establishments within our AOR
