“Feedback Forms/Suggestion Box/Openline

Communication”
Pandemic brought big changes in the lives of the people. As part of the
Learning Continuity Plan (LCP), modular distance learning was implemented to
deliver learning at home despite the threat of COVID-19. However, part of this new
system is the dilemma on how communication will be done in times of pandemic
considering the health and welfare of everyone.
Self-Monitoring Tool was given to the parents to serve as feedback of
information to the school on the status of modular distance learning. By this means,
the information written at the tool will be used to give better service to the community
members.
Moreover, a suggestion box was installed outside the school office to offer
opportunity to the parents to air out their views, suggestions, complaints or any other
information they consider necessary for the school administration. Open line
communication through online was also take into consideration to maintain
harmonious relationship with parents.

well as the suggestion box. Moreover, sample of Self-Monitoring Tools was also
gathered here.
