AFFIDAVIT OF CLOSURE OF BUSINESS

I, Josiah David F. Quising, of legal age, with residence and postal address at 1020
N Maxco Compound, Maries Village, Brgy. Pasong Tamo, Quezon City, after
having been duly sworn to in accordance with law, do hereby depose and say that:
1. I am the owner/representative of a business establishment named Q-OIL
GAS STATION, located at Purok 4 Brgy. Bacong, Ligao City, Albay,
Philippines.
2. I have been issued a Business Permit on June 28, 2016 by the Department of
Trade & Industry – Region 5 (Bicol Region) with Certificate No. 04116216.
3. On December 31, 2019, I closed said business due to bankruptcy.
4. I execute the foregoing instrument in order to attest the truth of the above
facts for all legal intents and purposes it may serve.
