NARRATIVE REPORT

March 13, 2023

On the first day of the Community Health Nursing duty, we went to LERC building,
Malaria, Barangay 185, Caloocan City for the orientation. In the orientation they discussed a few
things about the LERC organization, then after the discussion. Ma’am abi also orient us about
the things that we are going to do in the survey tool. First City Providential College conducted a
survey tool for guidelines about the community health needs in barangay last March 13, 2023
concerning the health and needs of the people in PWDs, LERC building, Malaria, Barangay 185,
Caloocan City. The survey which has provided us sufficient datas, the first part was to assess
number of deceased family member, education, employment, health, housing, income, support
services, and community out reach program.
The community is diverse group of people with different backgrounds, cultures and
health problems. Financial Constraints, smoking, alcoholism, lack of sleep, hypertension, pests,
and hospitalization of a family member. These factors if not given attention or not perceived by
the family may put them at risk of diseases. Each family has its unique set of health issues that
require attention and care. We as student nurses have a wonderful opportunity to practice as a
community health nurse at the LERC organization. Community Health Nursing is evidence
based practice care that focus on health promotion, disease prevention and maintain a healthy
living. Community Health Nursing involve assessment of a community, diagnosis of needs in the
community, planning how to increase health awareness in the community, implementing of the
plan and evaluating the outcome of the plan.
