VOTER’S ID (if not available or still not registered, please submit LATEST VOTER’S CERTIFICATE. If there is any discrepancy, pls submit Aff. Of
Discrepancy, if married, Aff. Of Change Status if your civil status in the document is not yet amended.

NOTE: If you still have active voting status, you can go to any COMELEC branch near your place. If your record has been deactivated or you are not yet
registered, or there is no COMELEC branch near your place, you can go to COMELEC INTRAMUROS BRANCH.

NBI clearance – no need to authenticate. Should have at least 6 months and above validity. All data encoded should be tallied in Birth and Marriage

LOCAL CIVIL REGISTRATION (also known as LOCAL LIVE BIRTH and MUNICIPAL REGISTRATION)
This record is available on your municipality (place of birth)
PSA BIRTH CERTIFICATE – must submit latest copy with receipt attachment. To all accounts under PREMIER broker, your Birth Certificate and Marriage
Contract should be verified by your agency.

NOTE:
- First Timers: All late registered for 21years old and above are no longer entitled to work in Taiwan or issue a resident visa.
- All discrepancies, amendment or any changes should be fixed before you apply to avoid future problem.
- ALL EX-TAIWAN who are late registered 21 years and above and granted resident visa in previous employment in TW are allowed.

LATEST PASSPORT - should have an 11 months and above validity and in good condition. Any damage, alteration or of any kind will result to visa
rejection.
OLD PASSPORT – all previous OFW’s in any country should pass said document

NOTE FOR EX-TAIWAN:

- Double check your entry and exit stamp, if incomplete, prepare your boarding pass and ticket copy for TECO’s reference. If you don’t have said
documents, go to TECO EMBASSY in Makati to apply Entry and Exit certification as replacement to incomplete entry and exit stamps.

ARC (Alien Residence Card) – this document is for EX-TAIWAN only. In case loss, provide an Affidavit of Loss with duration indicated (ex. AFF OF LOSS
of ARC 2015-2018)

PROOF OF STAY – all applicants from 30 years old and above are required to pass additional requirements as proof of stay. Said documents will start
after graduation. (ex. Graduated – 2002, docs needed to provide are from year 2002 up to 2018). Below are examples of documents:

 OLD documents with date indicated on it ex., nbi, brgy clearance, voter’s cert, receipts, water and electricity bills, or any documents you have
 BC of children if you have (may be local or psa)
 Marriage contract for MALE workers is considered

 Any documents with your name and date indicated
 Need to provide at least 1 document per year
Affifavit of Explanation – ONLY for EX-TAIWAN workers who stayed in Taiwan for less than 1 year

PEOS (PRE-EMPLOYMENT ORIENTATION SEMINAR – POEA requires all applicants to undergo this seminar online to help and prepare OFWs about
working abroad. Must print certificate for OEC application reference. (www.peos.poea.gov.ph)
E-REGISTRATION CERTIFICATE - POEA requires all applicants to input their data in POEA system to easily track record. Must print certificate for OEC
application reference. (https://eservices.poea.gov.ph)

NOTE: Submit all original documents needed and 1 set of photocopy. Latest passport should have 10 copies for other processing. Please provide long
plastic envelope for your documents.
