Tupas St., Cebu City, after having been duly sworn to in accordance with law, do
hereby depose and state that—

1. I was issued a business permit because of my business under the name

and style “A.L.O. Racing General Merchandise” for the year 2022;

2. Recently, I noticed that my business is not profitable anymore; thus, I

decided to close anymore;

3. In relation thereto, I have surrendered my business permit to the Business

Permits and Licensing Office to prove that I will be closing my
mentioned business;

4. I am executing this affidavit to attest the truthfulness of the foregoing and

for whatever legal purpose this may serve.
