I, Doblada, Mosez Anjelo Kian R. of Grade 8, Section Mendel, agree to comply with the necessary requirements in
the subject Research II, and fulfill the duties and responsibilities of a research student such as:
• attending to classes and submitting completed requirements on time
• avoiding acts of academic misconduct such as CHEATING and PLAGIARISM
• being prepared with appropriate materials and assignments for the subject
• showing respect to one another, especially during class hours
• resolving conflicts (group or individual) peacefully
• taking responsibility for my personal belongings and respect other people’s property
• behaving accordingly, without arguing, and cooperating when a member/s give/s direction or make/s a
request.
• voicing out my concerns at an appropriate time if I do not agree with the decision of the teacher and/or
group.
• avoiding out of town/country leisure trips during school days, unless necessary.
• not participating actively during lecture and laboratory classes
• keeping my parents/guardian informed about subject-related matters such as scores in quizzes, long
exams, submitted requirements, etc.
PARENT/GUARDIAN’S SECTION
I agree to help my child follow this agreement by:
• participating in any discussions concerning my child’s education.
• attending scheduled appointment/s if there is/are.
• informing the subject teacher for any important affairs that might affect my child’s
attendance/performance, by means of a formal letter.
• providing the subject teacher with active phone numbers and/or emergency contact information.
• understanding that certain appropriate actions might/will be given to my child for his/her wrong doings,
(e.g. tardiness, failure to submit subject requirements, cheating, bullying etc.) as indicated in the school’s
parent-student agreement form.
