How to apply for TES 2?
Prepare the following:
• ID for 4Ps and Listahanan 2.0 members
• Recent Income Tax Return (ITR) of both
parents / guardian
• Certificate of No Income from BIR (for
parents/guardian without ITR)
• Certificate of Indigency
Then wait for the announcement of application.

1- Wait for the announcement of available
scholarship slots to be posted at TUPC OSA
Facebook page.
2 - Download the application form at
http://bit.ly/TUPCSCHOLAR
3 - Print in a long bond paper.
4 - Fill out the Application Form.
5 - Complete the requirements listed on the second
page of the Application Form.
 How to apply for financial assistance?
LIST OF REQUIREMENTS:
• Grade 12 Card (for First Year Applicants) or
Evaluation of Grades (for old students)
• Endorsement letter from Shop Adviser (for old
students)
• Latest Income Tax Return of Parents/Guardian
• Latest Certificate of Registration

NOTE: Applicant must not have violated any minor or

major offense.
 How to apply for financial assistance?

5- To complete your application, email the

scanned copies / clear pictures of the application
form and requirements to:
tupc_scholarship@tup.edu.ph

6- Then wait for the schedule of your interview.
