4744 Old Sta. Mesa Street, Sta. Mesa, Manila, after having been duly
sworn to in accordance with law, depose and state:

1. That I am the father of RVIL RAIVEN V. BINUYA;


2. That Rvil Raiven V. Binuya is currently enrolled as Second Year College
student with a bachelor’s degree in Advertising and Public Relation at
the Polytechnic University of the Philippines, Sta. Mesa, Manila;
3. That my said daughter is applying in a Scholarship Program under the
CHED Student Financial Assistance Program;
4. That one of the requirements for the said scholarship program is a
proof of the Bureau of Internal Revenue (BIR) validated Income Tax
Return (ITR) of parents/guardian;
5. That I did not file any ITR at the BIR because I have been unemployed since
March 2005 and I have no other means of income or whatsoever in my name;
6. That I am executing this affidavit to attest to the truth of the facts and
statements aforementioned, for all legal intents and purposes it may serve.

1. That I was born on the 14 th day of June, 1973 to spouses Jesus


Morales and Felisa Morales and since childhood, I have been using the name
“IMELDA MORALES y MOLINA”;

2. That I am also known by my family and friends as “IMELDA

3. That I have been using the name “IMELDA MORALES y MOLINA”


in all my records and public documents;

4. That recently, I went to the office of Social Security System to apply


for an ID and to update my membership dues to which I was made to submit my
Birth Certificate;

5. That when I requested for a copy of my Birth Certificate from the


National Statistics Office, I was surprised that my name, as reflected on the
official document, was erroneously entered as “EMILDA MORALES y MOLINA”;

6. That I attest that the names “IMELDA MORALES y MOLINA” and


“EMILDA MORALES y MOLINA” refers to one and the same person;

7. That “IMELDA MORALES y MOLINA” is my true and correct


name;

8. That I am executing this affidavit in order to attest to the truth of

The undersigned requested for the revocation of her flexi-time schedule


which was subsequently approved by the Honorable Deputy Court Administrator
Raul Bautista Villanueva in his Letter dated June 16, 2014. The Letter stated
among others that the official time of the undersigned shall revert back to the
regular working hours of 8:00 A.M. to 12:00 noon and 12:30 P.M. to 4:30 P.M.
effective 1 July 2014.

However, the undersigned received the said letter only on July 24, 2014,
hence she was not able to adjust her schedule according to the regular working
hours as requested as of the date of its effectivity.

  I, JONIE G. MANZANO, of legal age, Filipino, and a resident of


Barangobong, Villasis, Pangasinan, after having been duly sworn to
in accordance with law, hereby depose and state:

1. That I am a duly-licensed driver in accordance with pertinent Land


Transportation laws, rules and regulations, and was issued a
corresponding Professional Driver's License;

2. That sometime on June 4, 2018, said driver's license was misplaced


and got lost;

3. That efforts were exerted to locate said driver's license, but in spite
of diligent search, it could not be found and the same is now beyond
recovery;

4. That said Driver's License has not been confiscated by the LTO, Police
or other Traffic Enforcers for any traffic violation;

5. That as such, I am executing this Affidavit of Loss to attest to the truth


of the foregoing and to support my application for the issuance of a
new Professional Driver's License, in lieu of the lost one.

PROPOSED ORDINANCE NO. 001 S-2020

AN ORDINANCE PROVIDING SUPPORT FOR PUBLIC SCHOOLS IN THE


CITY OF MANILA WITH COMPLETE TEXTBOOKS, AUDIO-VISUAL
EQUIPMENT, SCHOOL FACILITIES, AND OTHER LEARING AND
INSTRUCTIONAL MATERIALS

WHEREAS, Article XIV of the 1987 Constitution of the Republic of


the Philippines provides that the State shall give priority to education,
science and technology, arts, culture, and sports to foster patriotism and
nationalism, accelerate social progress and promote total human
liberation and development.

WHEREAS, Article XIV of the 1987 Constitution of the Republic of


the Philippines provides that the State shall protect and promote the right
of all citizens to quality education at all levels and shall take appropriate
steps to make such education accessible to all.

Section 2 par. (1) of Article XIV of the Constitution states that it is


the duty of the State to establish, maintain, and support a complete,
adequate, and integrated system of education relevant to the needs of the
people and society.

Title I, Chapter 2, Section 16 of the Local Government Code of 1991


provides that every local government unit shall exercise the powers
expressly granted for its efficient and effective governance and those which
essential to the promotion of the general welfare.

NOW, THEREFORE, be it resolved by the City Counsel of the City of


Manila as follows:

SECTION 1. TITLE – This Ordinance shall be known as the “Public


Schools Enhancement Program of City of Manila” or the PSEP Ordinance.”

SECTION 2. DECLARATION OF POLICY – The City, being the duty bearer


and main implementer, the decentralized actor in the claim of right of
education of its local constituents, must, therefore, give high regards with
the children as th e hope of our next generation.

It is the policy of the City of Manila government to provide its


constituents with basic educational materials in order to alleviate the
financial burden on the party of the indigent families who are bona fide
residents of the City of Manila. It is shall promote the right of every
individual to relevant quality of education regardless of age, sex, and
economic status. The City shall promote and maintain equality access to
education as well as to basic educational materials given to all public
school students.

SECTION 3. OBJECTIVE – The main and primary objective of this


Ordinance is to ensure that all public school students are adequately
prepared for the formal learning system that begins at kindergarten up to
high school. This covers the children/students that attended public
schools within the territorial jurisdiction and a bona fide resident of the
City of Manila.

While parents have the primary responsibility to provide for their


children’s physical, emotional, psychological, mental and social needs, the
local government service aims to supplement the inability of some parents
to provide their children with the proper care and home environment.

SECTION 4. DEFINITION OF TERMS – For the purpose of this Ordinance,


the following terms shall mean the following:

a) Public School – which refers to a school supported by government or by


public funds.

b) Learning Materials/References – which refers to educational materials


in tangible form such as but not limited to, textbooks, notebooks,
paper and pencils, reading and other instructional materials that will
serve as tools for the students in their physical, emotional, cognitive,
psychological, language and writing skills development.

c) Textbooks – books and other relevant educational materials for use in


public schools in the City. The ideal ratio per student (1:1) in all public
schools shall be observed.

SECTION 5. SCOPE OF BENEFITS – The beneficiaries under this


Ordinance shall be as follows:

a. Regular School Uniform

b. Physical Education Uniform

c. Prescribed Workbooks

d. School Bags with School Supplies

e. Text Books (Ratio 1:1)

f. Regular School Shoes

g. Rain Coats

h. Rain Boots

i. Hygiene Kits

SECTION 6. MISCELLANEOUS NEEDS – The City Government shall also


provide public schools with audio-visual equipment and instructional
materials such as television, CD, VCD, DVD, USB and others, as well as
other learning materials that will help ensure better learning for the
students.

SECTION 7. TECHNICAL WORKING GROUP – The Technical Working


Group shall be composed of the following members:

a. City Administrator representing the Office of the City Mayor


b. President, Liga ng Mga Barangay

c. Chairperson of the Committee on Education or his/her representative

d. Social Services Development Department (SSDD) which will serve as


the Secretariat

e. Chairperson of the Committee on Appropriation or his/her


representative

f. Division Heads or District Superintendents and all Principals of Public


Schools within the City of Manila

SECTION 8. PROCEDURE – Within sixty (60) days from the effectivity of


this Ordinance, the Technical Working Group shall be convened. The
SSDD shall submit the data with regard the number of the public schools
within the territorial jurisdiction of the City of Manila, and the actual
number of beneficiaries. The principals of each public school shall
determine the benefits to be distributed to their respective schools. They
shall resolve the manner and the timeliness on how the benefits shall be
delivered to the beneficiaries.

SECTION 9. REPEALING CLAUSE – All municipal ordinances,


implementing rules and regulations, and executive orders, or parts
thereof, which are inconsistent with the provisions of this Ordinance are
hereby repealed and/or modified accordingly.

SECTION 10. SEPARABILITY CLAUSE – If, for any reason or reasons,


any part or provision of this Ordinance shall be held to be
unconstitutional or invalid, other parts or provisions hereof which are not
affected thereby shall continue to be in full force and effect.

SECTION 11. EFFECTIVITY CLAUSE – This Ordinance shall take effect


immediately upon its approval.
