SUBJECT : After Activity Report on CHED Visit Evaluation of Second Year Level of
Government Permit for Operation of Bachelor of Science in Industrial Security
Management

The Commission on Higher Education Regional Office 5 through Dr. Jean E. Cardiño,
Education Supervisor specialist, visited the USANT Criminal Justice Education for Evaluating
the Second year Level status for Operating Bachelor of Science in Industrial Security
Management, so that appropriate Government Permit be issued before the opening of School
year 2020 – 2021.
The said Education Supervisor specialist; Dr. Cardiño arrived at about 10:40 in the
morning of March 6, 2020. And upon arrival the Dean College of Criminology Augusting C.
Camo entertained by offering snacks and after Dr. Cardiño started the monitoring evaluation
to see the compliance of USANT BSISM program to Program Standard and Guidelines under
CMO no. 6, series of 2018.
After looking for all the areas covering the Program Administration, Faculty,
Instructional Materials, Books, Laboratory Facilities and other Physical Facilities; the evaluator
finds that USANT BSISM is Compliant to the Mandates of CMO no. 6. But there are some
documents being required to submit on or before May 15, 2020; like; Notarized Appointment
of The Dean College of Criminal Justice Education, Qualified Program Head for BSISM, that
those who are not yet Master’s Degree Holder should to finish their Course Competency, to
arrange the titles per professional subject, and the OBEdized Syllabi and also the Standard
Institutional Prepared Curriculum.

And after the program monitoring evaluation, lunch was offered to the visitor. Report
was prepared and printed and copy of it was given to us for our reference and basis for
compliance and improvement.

And there, being no other matter to do the CHED Supervisor left at about 2:15 in the
afternoon.

Attached is the copy of the result of the said Monitoring Evaluation of Bachelor of
Science in Industrial Security Management for Second Year Level Operation this coming
School year 2020-2021.
