Pre-start / kick-off meeting/toolbox held with all crew and dive
team involved in the activity.
Vessel is securely moored with enough moorings / enough
anchor chain played out and the brake has been engaged on
the anchor winch.
Deckhand has been assigned, where required, to assist the
dive team and he has a working radio to communicate with
the bridge
All work that may conflict with the diving operations is
stopped until completion of the diving operations.
An observation post is in place to keep watch for other vessels
and small crafts approaching.
Prevailing water depth and draft clearance verified when
diving on / from a vessel.
When diving from shore, the diving area has been clearly
marked.
Emergency procedures have been discussed and
responsibilities are clear
Project Medivac procedure verified/transport available.

Communication
Communication line between dive supervisor and bridge
agreed and tested.
Flag “A” hoisted and visible from all directions.
Dive supervisor shall inform the bridge when the diver is ready
to enter the water and when he is out of the water.
Adjacent units / vessels notified.

Relevant authorities notified.


Communication devices with all persons directly involved (e.g.
signal holder, crane-driver, banksman and/or captain),
checked.

Isolation of systems and/or installations on the vessel: Applicable Yes / No
Main engines are isolated from the propellers (clutched out,
shafts secured, etc.) to prevent movements of the propellers
All propeller blades incl. (bow) thrusters disengaged. Main
engines by means of turning device and other thrusters by
means of electrical lock out/tag out (ER).
All suction intake and discharge valves closed and locked
(ER=engine room) in the vicinity of the diving operation.
(Depending on the location of the operations), applicable
pump or emergency FIFI pump put on standby and locked
out/tagged out (ER).
All ballast operations cancelled, and ballast pumps locked
out/tagged out (ER and deck department).
Interference of loading operations with the diving operation
checked (deck department).

All gates, valves, pipelines, intakes and discharges have been
identified and where required, isolated.
