    UNDERTAKING

    I, Son/ daughter of Shri


    resident of village/ town/ city district
    state hereby declare that I belong to the
    community which is recognized as a backward class by the Government of
    India for purpose of reservation in services as per orders contained in
    Department of Personnel & Training Office Memorandum No. 36012/22/93-Estt.
    (SCT) dated 08.09.1993. it is also undertaken that I do not belong to persons/
    section (Creamy Layer) mentioned in Column 3 of the Schedule to the above
    referred Office Memorandum dated 08.09.1993, as on 01.01.2012.

    I further declare, if subsequent verification reveals that my claim to belong to


    OBC or not to belong to creamy layer is false, my candidature may be
    terminated forthwith without any assigning any further reason and without
    prejudice to such further action as may be taken under the provisions of Indian
    Penal Code for production of false certificate.

    On basis of my undertaking as above, I may be permitted to join the Bank as
