• Procedure:
o Complete the online application for Polk State College to receive a Polk State College ID at
https://polkstatecollege.formstack.com/forms/application.
▪ When applying, on the 2nd screen where it asks “What is the education goal that best meets your
interests at this time?”, you should choose Option 4: I am a high school student and want to take
classes as a dual-enrollment student.
▪ The next question will ask if you qualify for early admission. The answer is No.
▪ When asked if you attend a collegiate high school, the answer is No.
▪ For “where will you be taking the majority of your classes” the answer is “on my high school
campus”.
▪ When asked for an email address, It is recommended that you use your My Polk Schools email
address as it should accept incoming emails from PSC without sending to spam. Check the email
address that you provided in the application process within 24 hours for emails from PSC with
directions on next steps including orientation and your PSC ID number. If you do not receive an
email from PSC, check your junk/spam folder. Forward a copy of the email with your PSC ID to
Mrs. Rerucha at stephanie.rerucha@polk-fl.net.

o Take the PERT test. You may sign up online for the next WHHS administration (April 20, 2021) at
winterhavenhs.net. Go to “dual enrollment” on the quick links and you will see the sign up link. If
you prefer, or if you miss the WHHS date, you can take the PERT remotely online through Polk
State. Here is the link to info: https://www.polk.edu/admission-aid/remote-pert-testing/
o Bring a copy of your PERT scores to Mrs. Rerucha for discussion of options and next steps. She is
available before school from 6:30-7 AM at the back desk in the main office or in her office after
school. Or you may email her to schedule a time to meet. At this point, if you qualify, you will be
given further instructions on selecting and registering for classes
o Complete the online orientation (you will need your PSC ID to do this step – it should have been
emailed to you). Here is the link to orientation: https://www.polk.edu/admission-
aid/admissionsteps/student-orientation/ Take a picture or screen shot of the final screen showing
completion and email to Mrs. Rerucha at stephanie.rerucha@polk-fl.net .
