Pg. 3 23/2/2023 Confidential Patient Name: SOMIGLI, Luca 1042322688 Dear Doctor, Reason for Attendance: 2\W upper GI cancer referral Summary You recently referred the above patient to the Gastroenterology department on a 2WW suspected upper GI pathway proforma, They presented with symptoms of abnormal scan and abdominal pain. ‘They were assessed by telephone and the following investigations were requested: Bloods: Slightly raised albumin (53) otherwise normal FBC, LDH, renal and liver profile. MRI abdomen and pelvis: There is gallbladder wall thickening in the fundus. No cystic change to suggest a diagnosis of adenomyomatosis and therefore the appearances are more suspicious. Hepatosplenomegaly. | note recent history of glandular fever. Transnasal endoscopy: DNA'd twice and then declined HPB-MDT review: Likely hepatosplenomegaly due to glandular fever, cannot definitely rule out malignancy but this seems unliekly - decided to discuss the case in the joined UCLH/RFH HPB-MDT. UCLH/RFH HPB-MDT: Diffuse thickening around GB wall. No CBD dilatation, no suspicious nodes. Abdominal ultrasound in 3 months advised and requested. We have reviewed HPB-MDT outcome in a clinic which advised a follow-up abdominal ultrasound in about three months. He is not keen on having endoscopic investigation for now. We are removing them from the ‘suspected cancer pathway. We will organise another appointment with our HPB team to follow-up on his gallbladder finding and define management plan accordingly.
