Innate Spellcasting. Iskalith’s innate spellcasting ability is


Charisma (spell save DC 21). She can innately cast the
following spells, requiring no material components:

At will: alter self, animate dead, bestow curse, confusion,


major image, modify memory, nondetection, sending,
suggestion

3/day each: greater invisibility (self only), mislead

1/day each: feeblemind, symbol (discord or sleep only)


Legendary Resistance (3/day). If she fails a saving throw, she
can choose to succeed instead.

Magic Resistance. Iskalith has advantage on saving throws


against spells and other magical effects.

Magic Weapons. Iskalith’s weapon attacks are magical.

Regeneration. Iskalith regains 10 hit points at the start of her


turn. If she takes cold or radiant damage, this trait doesn’t
function at the start of her next turn. She dies only if she
starts her turn with 0 hit points and doesn’t regenerate.

Ventriloquism. Whenever Iskalith speaks, she can choose a


point within 60 feet; her voice emanates from that point.

ACTIONS

Multiattack. Iskalith makes two Dire Hook attacks or makes


one Dire Hook attack and uses her Frightful Word once.

Dire Hook. Melee Weapon Attack: +9 to hit, reach 5 ft., one


target. Hit: 8 (1d8 + 4) slashing damage plus 16 (3d10)
necrotic damage. If the target is a creature, its hit point
maximum is reduced by an amount equal to the necrotic
damage it takes.

Frightful Word. Iskalith targets one creature she can see


within 10 feet of her. The target must succeed on a DC 21
Wisdom saving throw or become frightened of her for 1
minute. While frightened in this way, the target must take the
Dash action and move away from Iskalith by the safest
available route on each of its turns, unless there is nowhere to
move, in which case it needn’t take the Dash action. The
target can repeat the saving throw at the end of each of its
turns, ending the effect on itself on a success.
Teleport. She magically teleports, along with any equipment
she is wearing and carrying, up to 120 feet to an unoccupied
space she can see.

Twisting Words. Iskalith targets one creature she can see


within 60 feet of her. The target must succeed on a DC 21
Charisma saving throw or become charmed by her for 1
minute. The charmed target can repeat the saving throw if
Iskalith deals any damage to it. A creature that succeeds on
the saving throw is immune to her Twisting Words for 24
hours. She can only maintain this ability on one creature at a
time.

LEGENDARY ACTIONS

Iskalith can take 3 legendary actions, choosing from the


options below. Only one legendary action can be used at a
time and only at the end of another creature’s turn. She
regains spent legendary actions at the start of her turn.

Fire Shield. Iskalith uses the skull of a fallen enemy she


carries in her left hand to cast fire shield.

Corrupting Guidance. Iskalith uses Twisting Words.


Alternatively, she targets one creature charmed by her that is
within 60 feet of her; that charmed target must make a DC 21
Charisma saving throw. On a failure, she decides how the
target acts during its next turn.

Teleport. Iskalith uses her Teleport action.

Iskalith the Faint is a unique fiend who has claimed the


southern gatehouse of Tsar as her demesne. She uses guile
and trickery to deceive her foes while her magical charms
cause her enemies to fight each other instead of her.

Iskalith is largely based on Titivilus from Mordenkainen’s


Tome of Foes.
