Instruction: Put a check mark if the following are present and just leave if not observe (reference Region
Memorandum No. 215, s. 2015, Region Memorandum FTAD No. 8, s.2020).

1 A signboard/tarpaulin is posted, showing the following identification at the entrance of the
room:
a. Grade and section occupying the room.
b. Name of the class adviser

2 A framed copy of the approved class program is displayed on the door of the classroom at
adult eye level.
3 On the front wall (facing the class), the classroom chalkboard is located with the following
features:
a. properly framed:
b. with chalk ledge
c. with curtains; and
d. installed at a height which is in accordance with the maximum comfortable reach of
the children to the top of the board. (The proper height of the chalkboard from the
floor to its top-ledge is determined by multiplying the mean standing height of the
class by the constant 1.2)

4 Above the chalkboard, a framed portrait of the current President of the Philippines shall be
displayed at the center.
5 Flanked on one side at a lower level is a framed motto (for the month or week) and on the
other side by a framed picture preferably relevant to the motto. ( In Grade I and II, framed
perception strips are displayed along the top edge or frame of the chalkboard, while framed
conceptualizing strips are displayed along the bottom-edge of the chalk ledge)
6 The bulletin boards and tack boards, as well as charts, are placed on the walls at the sides or
at the back of the room.
7 The attendance chart and the DepEd forms rack are place near the door.
8 The teacher’s table and chair is located at the rear/side of the room.
9 The teacher’s cabinet is located at the rear/side of the room.

One corner of the room is set- up as reading corner with:
a. reading materials on it;

b. reading schedule/program; and
c. pictures of learners who used the reading corner during their vacant time.

Another corner of the room is set up as health corner with:
a. first aid or medicine cabinet with medicines;
b. mirror;

25 Above the chalkboard, a copy of the Pledge of Allegiance to the Philippine Flag is displayed
at the upper right corner.
26 Have a copy of the DepEd VMC.
A classroom-based data of the Basic Education Information System is placed on the walls at
the sides or at the back of the room with the following modules:
*Module A: Students’ EIS (Students’ Profile)
*Module B: Teachers’ EIS (Organizational Chart of DepEd Officials, School Officials, Profile
of Adviser & Subject Teachers.
*Module C:Curriculum Development ( K to 12 Basic Education Curriculum)

*Module E: Physical Facilities (Inventory of Classroom Property)

will be the one to accomplish the form with all his/her honesty.
2. After you finished accomplishing this form submit it to your principal or to assigned teacher for
validation.
3. Each item will bear 1 point. If the item composed of more than one criteria the total check will be
divided by the number of criteria.
4. After all items have been scored get the total score.
5. To get the final score divide the total score by 27.
6. And finally using the Key to Rating Scale below to determine the Descriptive Rating.

I. Physical Aspect of the Room (20%)
Average of the two Final Score or at least one (1) in the Classroom Structuring Self-Assessment

Note: For a relieving teacher they shall be assigned as co-adviser of a class where they have more
subject taught for them to get the score.

II. Mean Percentage Score (MPS) or General Percentage Average (GPA) (20%) (For adviser only)
Rubrics in scoring:
 If the quarterly MPS or GPA is increasing the score is 2.0
 If the quarterly MPS or GPA is fluctuating the score is 1.5
 If the quarterly MPS or GPA is decreasing the score is 1.0

Note: For a subject teacher the score will be based from the Average of all MPS or GPS of the
subject/s taught in the class where are assigned as co-adviser.Follow the same rubrics above to
get the score.

III. Daily Attendance (Based on SF2 – Learner’s Daily Attendance) (20%) (For adviser only)
Rubrics in scoring:
 If the average daily attendance is 95%-100% the score is 2.0
 If the average daily attendance is 90%-94% the score is 1.5.
 If the average daily attendance is below 90% the score is 1.0

Note: For a subject teacher the score will be based from the Average Daily Attendance of Learners
during their class on the subject/s taught in the class where they are assigned as co-adviser.Follow
the same rubrics above to get the score.

IV. Reading Performance (based on the result of Phil IRI) (20%) (For adviser only)
Rubrics in scoring:
 If there is an improvement in the reading skills of at least 50% & Up of the
non-reader/frustration or the combination from the base line at the end of the SY, the
score is 2.0
 If there is an improvement in the reading skills of at least 25% - 49% from the base line
at the end of the SY, the score is 1.5.
 If there is an improvement in the reading skills of at least 1% to 24% at the end of the SY,
the score is 1.0

Note: For a relieving teacher assigned as co-adviser of a class to get the score in this part they
should have have an approved intervention plan or action plan for the improvement of learning in
the subject taught for them to get a score. Follow the same rubrics above to get the score.

V. Classroom–based Action Research or Innovation (20%)


Rubrics in scoring:
 If AR is presented at least at school level, the score is 2.0
 If AR is conducted but not yet presented even at school level, the score is 1.5
 If proposed AR is not yet conducted, the score is 1.0

Note: For a research or innovations with complete write ups and submitted at least to the Division
Office for approval will automatically received a maximum score (2). It will no longer be used for
