1. Work Experience sheet / Updated Service Record

2. OTR,
3. Report of Rating / License
4. Copy of Training Certificate (4 hours minimum) relevant to the position

The LETTER OF INTENT shall be submitted either ELECTRONICALLY or thru

HARDCOPY to the office of the School Principal NOT LATER THAN Monday, October
17, 2022 and for purpose of initial evaluation (Q or DQ)
