Critical Thinking:

Over the course of this journal, time was spent on preparing and presenting a teaching
presentation on social determinates of health and well as a proposed project on how nursing and
nursing units can combat the problem of social determinant of health. For the sake of this
journal, I will not go into deep details of what social determinates of health are. Overall, social
determinates of health are factors that are factors that affect the outcomes of a person’s health.
Some of those factors include but are not limited to finances, transportation, education, location,
etc.
I have not had the opportunity to teach this block of instruction to the new graduate
nursing residency yet, however a date is scheduled for later in the calendar for this month.
However, my unit manager found out about my matriculation through graduate schooling and
the current project that I am working on and suggested that I have a small in-service class to the
new graduate nurse while they were orienting on the unit. After completing the in-service, the
overall effect of the class was underwhelming because the topic is something that has been
taught time and time again within the undergraduate curriculum. So, the question that I proposed
to them was “What can we do to combat factors of social determinates of health”. From this, I
was able to start a small project within our unit as well as help them create a topic for their
evidenced based project that is required for them to complete at the end of their nurse residency.

Based on a project completed with the previous cohort of new graduate nurses on our
unit, we were able to connect social determinates of health with the increased of patient
readmittances on our unit. The purpose of this project is to decrease the percentage of patient
readmittance on our specific unit through early screening of social barriers in health by the end
of the 2023 fiscal year. The overall acceptance of the project was positive from staffing,
encouraging the efforts, and demonstrates that staff is willing to be change agent in the known
issue of social determinants. This project helps to builds rapport between patient and facility to
help ensure that hospital is a haven for patient and that patients are more than just a number.
Pending the results of this project, we hope for the implementation as standard practice across
Emory University Healthcare System.

Personal Reflection:

This whole experience with my preceptor has been very eye opening and engaging,
especially when it comes to my career advancement as well as the opportunities that are possible
with me for this degree. From the beginning my lovely preceptor has been a driving force for me
to continue the progression of this program even when I was ready to give up. Her constant
motivation in the face of adversity allowed me to see the bigger picture and stay on course with
what was in front of me instead. She allowed me to see the value within this program and learn
how to adapt, manage, and find the beauty of what is in front of me instead of searching out for
a program less consuming and demanding. She balanced me focused me on the fact that the
struggles of todays will only make me stronger in the future. She allowed me to help identify my
reasoning for nursing as well as my purpose for continuing to pursue more in nursing. She
granted me the understanding to know things will never work out like how you expect them to
and for your current situation to improve, you must sometimes start from square one and go at it
again from a different perspective. Even though this practicum preceptorship was for education
and leadership, I received an amazing mentor and the true example of what a leader is and how
you should be in nursing when developing the future of the profession. For this I am forever
grateful for her taking time to continue to mold me into a resilient, thoughtful, and purposeful
professional.

Application:

The amazing mentorship that she gave me is something that I look forward to pass along
to those that seek mentorship from me, or even those who fall under my leadership and guidance
not only in life but their professional advances. The impact that she has on my developing of my
career in nursing is that impact that I want to pass along. When it comes to precepting new
graduates or even the education of nursing students or outside within the general community, I
want to have the impact of change. I want that impact to be one that allows people to focus on
what their purpose is and to always remember there why. I want the people I impact to
understand that it is perfectly fine to fail at something, so long as you get back up and aim for
the goal again. I want the people that I impact to understand that it is fine to acknowledge when
you have moved on something to early and understand that the regrets are nothing, but doubt
created. I want my impact on people to be the understanding that when you are uncomfortable in
a situation, it is because a change is needed.
