NOTICE OF WITHDRAWAL

退學申請

Please note that two month’s written notice is required if you intend to withdraw your child
from the school. Please ensure that you fill in this form and pass it to the school
administration, or provide the school with other written notice of withdrawal, TWO
MONTHS before your child is due to leave. The notice period will commence when
administration receives this withdrawal form or other written notice of withdrawal.

請注意辦理退學手續需於兩整個月前以書面形式(如電郵/信件,或填妥此表格)通知本校,家長需繳交學費
至申請後第二個月的月底(例如:於五月一日或五月內任何日子通知本校辦理退學,需要繳交學費至七月
底)。校方會以收到書面通知的日期計算。
