That we both personally know JUDY LYN CARANDANG PUYOS, of legal

age, married, born on May 20, 1985

That her name appearing on her Voter’s Certification issued on August 23, 2006
with Voter’s Identification No. 10050534AE2085JPG20000-8 there in is JUDYLYN
PUYOS GAZO; also her name appearing on her son’s birth certificate named Kean
Jacen Puyos Gazo with Registry no. 2012-1608 there in is JUDY-LYN CARANDANG
PUYOS; appearing on the Certificate of Marriage with Registry No. 2011-399 issued by
Municipality of Bauan Batangas there in is JUDY-LYN CARANDANG PUYOS; and
that her name that appearing on Certificate of Live Birth with Registry No. 85-2507
issued by Municipality of Batangas there in is JUDY LYN CARANDANG PUYOS is
one and the same person;

We are executing this affidavit to confirm and affirm the foregoing facts and for
whatever legal purpose it may serve.
