JOINT AFFIDAVIT OF CONSENT

TO TRAVEL ABROAD

That we, AVELINO E. SUMUGAT and MA. RIADEL U.

SUMUGAT, both of legal ages, Filipinos, husband and wife,
respectively, and residents of Purok 1, Villarica, Midsayap, Cotabato,
after having been duly sworn in accordance with law, do hereby
depose and say:

That we are the natural parents of EMMANUEL RAVE U.

SUMUGAT, who was born on January 29, 2001, at Cotabato City,
and 17 years of age;

That my son Emmanuel Rave U. Sumugat and Grade 12

student of Southern Christian College, Midsayap, Cotabato;

That we are giving our consent that our son Emmanuel Rave U.
Sumugat that he will travel abroad at Bexco, Busan, South Korea
together with Efresela Leer Mejia, President of International Youth
Fellowship Davao (IYF) to attend the 2018 IYF World Culture Camp
on July 15 to 26, 2018;

That we are giving our consent to our said son, Emmanuel Rave
U. Sumugat as part of his exposure and leadership training as an
officer of his school;

That we execute this affidavit to attest to the truth of the

foregoing facts and for whatever legal purpose the same may serve.
