1.0 Objective
1.1 To determine whether the applicant has the physical requirements required to be able to perform the usual
requirements of the job;
1.2 To ensure that the applicant has no pre-existing medical condition that would prevent him or her from
performing essential tasks;
1.3 To advice individuals in the maintenance or improvement of their health;
1.4 To establish a baseline record of the general health condition of the individual.

2.1 This policy applies to all CMC qualified candidates for employment

4.0 Policy Details


4.1 The following procedures are recommended in order to bring about consistency, effectiveness and efficiency in
the conduct of pre-employment medical examinations.
4.1.1 All qualified candidates are required to undergo physical and medical examinations and shall be
informed by the Recruitment Staff as part of their Employment Requirements Checklist (see Annex “A”). The
applicants shall also be advised to bring one (1) 2x2 ID picture for the application.
4.1.2 All qualified candidates are only given two (2) working days (upon receipt of the endorsement from
HR) to complete their physical and medical examinations. Exception: Female candidates with monthly period are
still advised to complete their physical and medical examinations except for the submission of urine sample for
Drug Test. Submission of the pending urine sample shall be made within five (5) working days upon completion of
the physical and medical examinations. It is the responsibility of the applicant to inform the Recruitment
Officer/Staff and the Officer-in-Charge of the Industrial Department about her condition.
4.1.3 Payment for physical and medical examinations shall be shouldered by the qualified candidates. All
payments shall only be made to CMC Cashier in the amount of Php 800.00 for male applicants and Php 950.00 for
female applicants.
4.1.4 The Official Receipt shall be forwarded to the Industrial Department. Pre-employment Physical and
Medical Examination Application Form (see Annex “B”) shall be filled up by qualified candidate and affixed his/her
latest 2x2 picture on the space provided. He/She must also sign this form and attest by the medical officer/
examining physician afterwards.
4.1.5 The form is intended to provide the information to the medical officer/examining physician, which is
crucial for the proper conduct of medical examination.
4.1.6 The following tests are recommended for all applicants (for all positions):
4.1.6.1 Medical History
4.1.6.2 Ophthalmological Examination
4.1.6.3 Otoscopy
4.1.6.4 Dental
4.1.6.5 Physical Examination
4.1.6.6 X-ray and Laboratory Report
4.1.6.6.1 Chest X-ray
4.1.6.6.2 Complete Blood Count
4.1.6.6.3 Urinalysis
4.1.6.6.4 Stool Examination
4.1.6.6.5 Sy-Serological Test
