AFFIDAVIT OF CONSENT

I, JOHN CHRISTIAN ANTHONY D. DOLINA, of legal age, and a

resident of 208 Second Street, Sampaguita, Tacloban City, Philippines,
after having been duly sworn in accordance with law, hereby depose and
state:

1. That I am the biological father of minors, KRISHEL CHIARA

XYLEN LACABA DOLINA, 10 years of age and JOHN YANZEL LACABA
DOLINA, 7 years of age with RICHEL VILLAMOR LACABA, who are going
for a trip abroad;

2. That as a loving father who cares for the welfare and well-
being of my children, I am giving my full consent to KRISHEL CHIARA
XYLEN LACABA DOLINA and JOHN YANZEL LACABA DOLINA to travel
abroad or specifically to Sydney, Australia on June 2012;

3. That further, my children will be accompanied or escorted by

their biological mother RICHEL VILLAMOR LACABA in their travel
abroad and shall be the one to financially support them in their travel
abroad; and

4. That I am executing this affidavit to declare the truthfulness

of the foregoing facts and for whatever legal purpose this may serve.
