This letter is to request assistance for medical treatment on behalf of Mr.. , 71 years old,
a resident of ___, who is currently confined at of hosp.. He has been diagnosed with
Covid 19 last April and is still battling the virus in the ICU

I am humbly requesting for your assistance so we can help Mr. __ because sustain the
hospital expenses that are too expensive for him to afford given his senior status with
no income. I understand that you give assistance to people who are eligible by helping
them pay for their treatment to battle the virus. 

I have on hand his medical transcript and will enclose a copy of it here. I have also
enclosed a letter from his doctor stating the medicines required for him to continue his
treatment.
