1. INTRODUCTION
1.1. General
PT AGN has a GTU Reboiler item which is found to have a visual indication in the form of
wall loss due to friction from the support. Internal Rotary Inspection System (IRIS) is the
UT method used by owner to check the indication. PT ROBUTECH was appointed by PT
AGN to carried out inspections using this method. In the first stage the examination was
carried out randomly from row 1 until row 3 on April 16, 2022, the results of 70,73% of
examinations found that there was wall loss due to friction of the support tube more than
50% of the original thickness. Engineer of PT.AGN decided to carried out a further
inspection with the same method to find out to what extent these indications occurred
on his equipment based on the results of the first inspection. This report contains the
results of the inspection and the actual condition of the equipment.
1.2. Objective
The purpose of this examination activity was to determine the condition of tubing and to find any
anomaly(es) such as, general corrosion (ID/OD), pitting corrosion (ID/OD) or/and defect under
baffle/support plate (OD) that may be occurred due to the service conditions and caused wall
thickness degradation and to find out the extent to which the indication of wall loss occurred on
this equipment

2. SCOPE OF WORK
2.1 CLEANING
Result of the IRIS method be affected by the cleanliness of the inside of the tube so that
during this inspection cleaning activities are carried out to accommodate the IRIS
inspection so as to get good results. The details are as follows:

8. RECOMMENDATION
There are some recommendations based on the analysis of inspection results that have been carried out:
8.1. The results of the inspection using the IRIS method are influenced by the cleanliness of the inside
of the tube so that a more proper cleaning method is needed so that it can produce maximum
results. The mechanical cleaning method can accommodate the cleaning of this equipment, but
there are some tubes that require other cleaning methods so that the user can choose other
methods such as hydro blasting which may be combined with the currently used method.
8.2. Acceptance criteria for each tubing inspection have never been stated to national or
international standards or codes, so the recommendation to be plugged or replaced is purely
from the point of view of the user’s engineer that considered some factor such as, service
conditions, corrosion rate and service life.
8.3. Thickness reduction (wall loss) due to and corrosion and friction between the baffle plate and
the tube may occur repeatedly if root cause is not found. Deep analysis may be carried out to
find the root cause and how to minimize the occurring of this damage mechanism.
8.4. Inspection program should be carried out periodically as monitoring based on result of previous
inspection for monitoring.
8.5. The inspection technique, observations, and NDE method that carried out on this inspection
should be conducted periodically in the monitoring purpose, as per previous inspection
technique, observations, and NDE method.
