Items to be checked:
1. Visual inspection from splash zone up to top of platform structure to verify structural integrity and
equipment layout, any modification on the deck shall be reported (recorded by sketch & photograph).
2. Visual inspection and video swim under surface control from splash zone down to seabed to verify
structural integrity and photographic documentation (if required).
3. Anode survey
Anode mapping shall be performed and estimates of depletion recorded, dimensional check of anode shall
be performed randomly (if required).
4. Scour survey
Scour beneath the bottom plan shall be determined and recorded at all legs & 3 locations (both end and
centre) up to radius of 20Ft. around jacket vicinity (recorded by sketch).
5. Debris survey
Any debris in contact with the structure and debris survey out to 20Ft. distance from jacket (recorded by
sketch).
6. Riser survey
All risers shall be surveyed from seabed to water surface for signs of damage.
Bolts and clamps of risers shall be inspected for sign of damaged, loose or missing at water surface down
to seabed and riser elbows on seabed shall be inspected, extended minimum 20Ft. off J-bow (recorded by
sketch).
7. Cathodic Protection Reading (all legs and outside only)
Reading carried out at splash zone and all nodes from water surface down to seabed.
8. Marine growth measurement, at Leg A2.
- Measurement starts from Msl. down to first elevation in every 5Ft. interval.
- Measurement taken below first elevation down to seabed shall be taken every 15Ft. interval.
- Photograph documentation may be required if deemed necessary for evidence and cleaning record if
any to be reported.
9. Thickness measurement, at Leg A1.
- Measurement shall be taken as near to the sea surface as possible for all members passing through
the sea surface.
- Measurement shall be taken at all legs or at least at leg A1, from sea surface down to seabed at leg
nodes and midpoint between elevations (taken at outside part).
10. Conductor survey (if any)
No. and location of all conductors to be recorded at splash zone down to seabed.
Outboard conductors that are welded to structure should be assessed corrosion and integrity.
Visual check of all bolted connection for damage or looseness.
11. Caisson supports (if any)
Visually check for weld cracking at splash zone down to the end of caisson.
12. MPI and CVI
- CVI and MPI at -30Ft. elevation Joint J273(262-273), no crack indication was found.
- Vertical Diagonal Member at Row “1” face “B1-A1” was selected for close visual inspection.
- No mechanical damage found at the VDM, good condition.

FIELD REPORT

SUMMARY OF INSPECTION RESULTS

GENERAL VISUAL INSPECTION

• The below sea level a general visual inspection was carried out with an underwater video
camera and surface monitoring capability from MSL down to -80 ft. elevation.
• The general inspection of the structural on the caisson, piles/legs and conductors revealed no
significant physical damage or defect, at the time of inspection.
• General visual inspection below sea level conducted from MSL down to the seabed, no
significant physical damage or defect observed.
• The construction integrity of boat landing and connection points to the structure had found in
good condition.
• The general visual inspection of this structure at above sea level, were carried out by the on
board of the PT. Radiant Utama Interinsco Surveyor.
• The underwater general visual inspection was carried out with an underwater video camera from
MSL. down to the seabed.
• The inspection of the structural member below sea level revealed no physical damage or defect.
• The all conductors and caisson were visually inspection for physical damage and looseness, no
physical damage was found.
MARINE GROWTH MEASUREMENT
• Leg A2 was selected for marine growth measurement.
• Measurements were taken from MSL. down to 1st level at -30 ft. elevation in 5 Ft. increments,
and 15 Ft. increments below 1st level at -30 ft. down to seabed at -75 Ft. elevation.
• The maximum thickness of hard marine growth was recorded 4.00 inches at -10 Ft. down to -
60 Ft. elevation and maximum thickness of soft marine growth was recorded 2.00 inches at -
10 Ft. down to -30 Ft. elevation (refer to the marine growth table).
• The photograph of marine growth was carried out at -30 Ft. elevation at the time of inspection.
CATHODIC PROTECTION SURVEY
• The cathodic potential measurements were taken on all legs of this structure, by using
Bathycorrometer BUCKLEYS Serial # BUC-C8110E and calibration carried out before and after
each dive.
• All cathodic potential readings fell between -0.922 volts to -0.973 volts at the time of inspection.
• Total 62 anodes were visually inspected, with the result 9 anodes were noted missing (leg only),
19 anodes were noted 100% depleted (frame only), 34 anodes were noted 20%-90% depleted,
3 anode rack pods were noted 75% depleted.

DEBRIS SURVEY
• During the general visual inspection, debris survey carried out on this structure from MSL.
down to the seabed at -80Ft. elevation.
• No metallic debris was observed from MSL. down to seabed, at the time of inspection.
• Large quantities of metallic debris were noted scattered on the seabed area, at the time of
inspection (refer to debris table).

SUMMARY OF INSPECTION RESULTS: (continued)

ULTRASONIC WALL THICKNESS MEASUREMENT

• Leg A1 selected for Ultrasonic Wall Thickness measurements.
• The measurements taken from MSL. down to the seabed (refer to the Ultrasonic Wall Thickness
measurements table). The Ultrasonic Wall Thickness measurements taken by using a
CYGNUS-1 Serial #3330 and calibration carried out before and after each dive.
• The highest reading of the inspected leg wall thickness recorded was 38.3 millimeters; this had
been recorded at MSL., the lowest reading obtained was 13.0 millimeters; this had been
recorded at -55 ft. elevation.
SEABED SCOUR SURVEY
• Survey of the seabed area enclosed by the structure and immediate surrounding areas observed to be
fairly even.
• The maximum scour under all horizontal mud braces was noted 4 inches on leg A1 row “1” face “A1-B1”.
• The riser no. 1 under riser of J-bow was noted 1 feet 3 inch scour on seabed and the rest are touching to
50% buried in seabed toward 20 feet the pipeline.
RISER INSPECTION
• 1 (one) of riser had been installed on this structure.
• The riser was visual inspected with an underwater camera and surface monitoring system from MSL down
to seabed and 20 feet outward the pipeline.
• The riser appeared to run parallel to the jacket structure and no mechanical damage could be observed.
• All bolts of the riser clamp were checked for integrity and tightness. Total 3 bolts are loose and no missing
bolt, at the time of inspection.
CLOSE VISUAL AND MAGNETIC PARTICLE INSPECTION
• 1 (one) member was selected for Close Visual Inspection, it was VDM member at Row 1 Face B1-A1,
no mechanical damage was found, good condition.
• 1 (one) weld joint was selected and carried out for Close Visual and Magnetic Particle Inspection.
• The digital photographs were taken at overall 12, 3, 6 and 9 O’clock positions of selected weld joint.
Weld joint number: J273(262-273) at -30Ft. elevation.
- The complete clock rotation of the weld and HAZ area were visually inspected.
- No crack indication was found on this weld joint.
