Assessing the Skull and Face

1. Introduce yourself and verify the client’s identity. Explain to
the client what you are going to do, why it is necessary, and
how the client can cooperate.
2. Observe appropriate infection control procedures.
3. Provide for client privacy.
4. Inquire if the client has any history of the following:
 Lumps or bumps, itching, scaling, or dandruff
 Loss of consciousness, dizziness, seizures, headache, facial
pain, or injury
If so, ascertain the following:
 When and how any lumps occurred
 Length of time any other problem
 Associated symptoms, treatment, and recurrences
5. Inspect the skull for size, shape and symmetry.
6. Palpate the skull for nodules or masses and depressions,
 Use gentle rotating motion with fingertips. Begin at the front
and palpate down the midline, then palpate each side of the
head.
7. Inspect facial features.
8. Inspect the eyes for edema and hollowness.
9. Note symmetry of facial movements.
 Ask the client to elevate the eyebrows, frown, or lower the
eyebrows, close the eyes tightly, puff the checks, and smile and
show teeth.
10. Document findings in the client record.
