PARENTS CONSENT/PERMIT/WAIVER

To Whom It May Concern:

This is to certify that I/we, the undersigned parent/guardian have given full consent and granted
permission to my granddaughter, THESSALONICA MARIE S. DE LARA ,taking Bachelor of Science in
Cruise Ship Management at John B. Lacson Foundation Maritime University-Molo to undergo an actual on–
the-job training in PANABOR RESTAURANT at Festive Walk Mall, Mandurriao, Iloilo City,
Philippines as a requirement in her enrolled subject Practicum-Work Integrated Learning (150-hr).

I/we am/are fully aware of the advantages and the benefits of which my son/daughter can acquire
from this undertaking specifically to hone her potentials to the fullest.

That I/we voluntary waive any claim against the University, the above-named Company, and its
authorities in-charge for any untoward incident beyond their control which may occur in the course of her
participation on the said on-the-job training, after all precautionary measure and exhaustive efforts has been
taken by the person-in-charge.

I acknowledge that I have read each and every one of the provisions in this consent/permit/waiver, and
I understand each of the stipulation and I agree to abide by them.
