Present entire LE. Snip learning objectives. In the annotation, explain what research-based knowledge
and principles of teaching were used or you can relate to the objectives. (Use one page para di masakit
sa mata) Snip instructional activity/s. Explain what research-based knowledge and principles of teaching
were used or you can relate to the instructional activities. ( Use another page). Do the same with
assessments.

Rating of 4-VS ( kapag 2 component lang example objective lang and assessment ang nakaanchor sa
research and principles of teaching)

MOVs – accomplishment report of remedial with list of least/most mastered skills and frequency of
errors over correct responses. (Lagyan din ng annotation to explain what happened)

MOVs – intervention materials made and used with list of least/most mastered skills and frequency of
errors over correct responses. (Lagyan din ng annotation to explain what happened)

Rating of 3 – Planned teaching and learning strategy/program

Objective 13
Rating of 5 – Collaborated with community stakeholders on the implementation/completion of program
that maintains a learning environment responsive to community contexts with accomplishment report

Rating of 4 – Planned with community stakeholders on the implementation/completion of program that


maintains a learning environment responsive to community contexts with Program/project/activity
plan

Rating of 3 – Conducted a consultative meeting with community stakeholders on the


implementation/completion of program that maintains a learning environment responsive to
community contexts with minutes of the meeting

(Kung kailangan ng annotation po lagyan to explain how learners learned from the activity, this is
community contexts)

Objective 14
Dito po gagawa kayo ng annotation to show your improvements through reviewing of personal teaching
practices. Dapat mairelate nyo sya sa Code of ethics for Professional Teachers specifically Article 8 – The
Teacher and the Learner

Rating of 5 – annotation of video or audio recording. (attach the video or audio recordings to show
improvements)
Rating of 4 – annotation of teaching materials (mag-attach din to show the improvements)
Rating of 3 – Personal reflection notes (mag-attach din to show the improvements)

Objective 15
Dito lahat tayo meron dito since lahat tayo kasama sa F2f implementation
As proof of participation, may ginawang certificate si Van, hingi na lng kayo ( change na lng name and
position, example Face-to-face class adviser). Rating of 5 na ito since di ka naman bibigyan ng cert if you
were not able to sustain the participation.

MOV- LE with annotations (ipapaliwanag kung anong philosophy of teaching that is learner-centered ang
naging basis/bases sa paggawa ng learning objectives, instructional activities and/or assessments
(components of instruction)

Objective 17
Documentation of feedback that shows that you have exhibited a respectful and caring attitude and/or
integrity to different stakeholders.

Objective 18
MOV – Updated development plan from Phase II (goals are already updated as a result of the midyear
review ( mag-annotate na lng din siguro kayo dito to explain)

Objective 19 (Plus factor)


Rating of 5 – beyond the school community (example: Division, Region, National)
Rating of 4 – within the school community
Rating of 3 – withing the learning area or department (English, Math..)
