3) Forms in this file must be completed with blue ink based on the regulations, all
stamps and signatures must be completed. The file shall be returned to Mr.
HAKAN AYDIN’s Office(Ext:15283) uponcompletion of the training.
Copies of license, logbook and valid medicalcertificate must also be included in the file.
4) Unless otherwise stated by the instructor, trainee must be ready at the briefing
room 1 hour before the simulator flight.
5) Recurrent Training Only: Trainee holding a CATII/IIIB certificate will complete Low
Visibility Recurrent Training at the end of the skill test. “Certificate of Low Visibility
Operations” Card may befound in the SIM folder obtained from B777 Training
Supervisor’s Office. The card will be signed bythe TRE conducting the Check.
• LPCs (Annual recurrents); 6 months after the license expiration date,
• OPCs (6 months recurrents); same as the license expiration date.
6) Recurrent Training Only: For LPC Trainings only, licenses of the trainees must be
stamped and endorsed by the examiner. For OPC Trainings, the examiner will not
stamp the licenses.
7) The days with the code DSM on your rosters, are planned the day before the OPT/LPT
simulator. These days are for the trainees to review the procedures and items on the given
syllabuses with their simulator buddies. The review shall be done either in the Training
Center or another suitable place.
*Simulator training and check forms need to be downloaded and used from DDMS only.
