Call to Order: Meeting was called to order at XXX Ground.

The meeting was started with the singing of Philippine National Anthem, Sugbo Hymn, Prayer and
then the welcome address by Mrs. C. F. Cruz, the Guidance Designate of XXX National High School. Comes
next was the introduction of our new School Head Mr. G. L. Lee

Sir Lee highlights the following:

a. The start of Limited Face to Face Classes is on March 28, 2022. XXX NHS is included in the
Division of YYY List of Schools for F2F Classes who belong to Alert Level 2 based on the
assessment conducted.

b. The school had coordinated with the baranggay officials and Sir Lee (the School Head) was done
attending the Barangay Session last March 11, 2022. The barangay captain had already signed the
resolution as well as the municipal mayor. Each learner must submit the written consent stating
that their parents are allowing them to join the LF2F.

c. Learners are allowed to stay in school within 4 hours/ half day for two days and recess is allowed
inside the classroom.

ORIENTATION PROPER

With Mrs. Lea S. Park Time Scheduling and Groupings were tackled.

a. Afternoon shifts are allowed to schools with high number of enrollees. High number of enrollment
are divided into 2 groups Set A & Set B. Subject teachers will meet the learners once only per
week.

b. Assignment & other activities will be given to students to be answered at home.

c. Morning shift will start at 7:30-12:00; afternoon shift will start at 1:00-4:30 with maximum of 20
learners

Mrs. Shella L. Cho widened the understanding on Health Protocol to be observed intently by
everybody.

a. Following of direction on Entering School Premises, Health Monitoring, Entrance/Exit,


Handwashing Station, Dis-infection & Health Check in every learner.

b. May Go Home Slip for learners who experience symptoms/ health related concerns. It contains the
contact number of the parents/guardians in case of emergency.

c. Health Form to be submitted by the parents which states that within 24-72 hours, there are none
of the family members who were diagnosed with COVID-19.

d. Wearing of gloves of learners if wanted

e. Wearing of face mask & face shield

f. Avoid eating in public places

g. Learners must bring mini alcohol dispenser/container

h. Extra face mask, alcohol, ball pen ,shoe mop, tissue & paper
Other Concerns:

a. Attire to be wore by the students, White T-shirt

b. Clearing & Cleaning of DRRM Personnel, Barangay Officials and Parents/Guardians to Provincial
Building which was affected by the typhoon Odette.

c. List and information about the students per Set will be posted and disseminated on March 18, 2022.
