Affidavit of Financial Capacity

I, Shamille Eadie R. Lescano, 16 yrs old, a resident of Brgy. 2, Lipa City, do hereby solemnly
affirm and declare as follows:

1. That I am permanent resident and citizen of the Republic of the Philipines by birth. 

2. That I am an applicant for the Senior High School Voucher Program under the Department of
Education, in partnership with SULONG EduKALIDAD, and the Private Education Assistance
Program (PEAC);

3. That I am dependent to the income of my Father, Ariel Lescano;

4. That, having three (3) other siblings who are also studying, my father might not be able to
support us and will add to the financial constraints of the family;

5. That I am executing this Affidavit as part of the requirements for the application of the Senior
High School Voucher Program;

6. I am executing this affidavit to attest to the truth of the foregoing and for whatever legal
purpose it may serve.
