size white bond paper of not less than substance (80 gsm) thickness. Photocopies of large documents
shall be reduced accordingly. Each set shall be submitted using a BLUE CLEAR BOOK FOLDER,
COMPLETE WITH TABLE OF CONTENTS AND TABS.
2. Submit a softcopy of all the completed application documents in item 1 above and all supporting
documents. Include an index file of all documents.
3. All pages of the application must be signed and dated by the applicant and shall be delivered or mailed

Personal engineering and management contribution. My 2 years


experience in construction of Gmall Digos in 2014 the DSG & Co. Inc. Hired
me in January 14, 1996 after that I turned over the Apo view hotel, tower
inn and casa Leticia hotel to my boss engr. Jing Abella and engr. Romeo
Gravador. I work as project engineer at the start. I work 24/7 at the site
after six month, my boss Arct. Merafuente & Engr. Lary withdraw the
project as project management group that monitor the on going activities
of contractor that monitor work schedule. My work is to monitor the
delayed pouring schedule of concrete, the steel fabrication for columns &
beams. The form works and the delivery of concrete to form works and the
delivery of concrete the site from concrete plan of Vic Lao then to Mr.

responsibility that my former boss left the project. I am now the one taking
in charge of the construction; supervise the weekly meeting with the

2. Problems faced during the project/activity duration

Review the accomplishment report, study the report that submitted to


us by the contractor, evaluate the cause of delay of the project. Evaluate
and review the change of design cause by the availability of materials
especially the reinforce bar. Review the drainage plan accomplishment,
the electrical, the problem in other side of the property. In case of form in
different side of the area especially during rainy days assert the injuries or
injured workers. Review the different incident in working area where the
accident happened. Review safety plan of the committee on safety and
health. Get the suggestion of the personnel and workers. Make some
advices on how to cope the up to date schedule. Review the schedule on
scaffolding installation, the fabrication of form works, the update of
installation of reinforce bar for flooring, beam, girder and columns. Then
the schedule of removal of forms for columns, beam, girder and flooring
then ask the concrete supplier for them. The quantity should be maintain
then the 3 day consumption of concrete report for the billing, review the
billing of aggregates sand & gravel, the chb and the boulders payments of
form materials the plywood the coco lumber. The supply of c.w walls, the
wire, cord structural paste epoxy, the paint, and the supply for plumbing.

Solutions formulated/ found and implemented – I asked the owner


about the need to have a fire fighter volunteer for the Digos Gmall
response team and i asked the permission to Filipino Chinese fire brigade
and ask guidance regarding the fire fighting training and seminar including
the water connection, potable water supply connection and possible
additional design of cistern facilities at basement of Gaisano mall of Digos
along Tres De Mayo, Digos city. The Filipino and Chinese fire fighter
instructor with other personnel showed me their fire fighting network
drawing plan where the possible connection to be cut off and connected.
They also shown to us guide for fire fighting training the hour needed
every week end at least six hours every Saturdays and holidays. I
suggested to the training structure to include the lecture of safety and

of the Philippines i suggested that a treated waste water from recycle
waste facility to used during the actual fire fighting in case of fire because
of limited resources of water it is need to minimize water extraction an

by regulating effluent discharge from any establishment and mandated for
the management and protection of water resources in the Philippines. The

appropriate treatment technology that are cost effective. In response to
this, it was necessary to establish a waste water treatment and recycling
plant on facilities that is efficient in reducing pollution and yet less costly
in terms of capital investment and operation. In view of this, a 300 cu.
Meter per day trickling filter – rotating biological contact (t-rbc) system is
set as secondary treatment to further treat existing grease trap effluent
waste water generation – the Gaisano of Digos also known as g-mall is a
major shopping mall located at tres de mayo, Digos City Davao del Sur it
is one of largest mall in Digos, the 300 cu. Meter per day capacity of the
rcb system is bades on waste water generated on mall activities. Brief
description of waste water treatment process. Sump pit – effluents are
collected in this unit. Pre – treated waste is then pumped (lifted) to the 3
stages grease trap tank. Inside is a submerged pump interconnected to a
level switch which automatically runs whenever level critical. Pollution

the rate 300 cu. meter per day. A transfer pump with sufficient head is to
be used for distribution of product water to desired location of use. The fire
fighter personnel instructed me to submit the drawing plan proposal prior
to their approval. The drawing plan proposal I submitted was incorporating
the fire fighter requirements to include to the proposal like flow meter
assembly which also requirement for environmental management bureau
of department of environmental office I have provided all the necessary
drawing plan. The connection detail the sanitary connection detail shown
the difference of invent level between the projects drainage line and the
existing network.

4. Description of Engineering judgements made

The Engineering judgement that I made was based on my previous project


experience that prior to start on of any project construction we should get
first all necessary requirement to start with. 1. Environmental compliance
certificate 2. Traffic impact assessment 3. Get a permit from DPWH district
for the tapping of a drainage system 4. Zoning certificate for commercial,
light heavy industrial and heavy industrial zone 5. Barangay clearance
interposing no objection of structure that may we built in their area of
responsibility 6. Relocate the informal settler affected of the project
construction 7. Coordinate to city engineer for possible demolition of
structure if needed 8. Make request for cutting of trees in the area where
the project is located by the agency task to do like DENR 9. Get the
topographic survey for the drainage elevation slope for requirements 10.
Apply for temporary water connection for electricity 11. Apply for
temperature electrical connection for electricity 12. Produce the vicinity
map then to start planning of the structure to be putted in the area. We
prepare the engineering plan, drawings of different for all discipline that
compromised a building structure like A. Site development plan signed by
a geodetic Engineer B. Architectural plan signed by architect C. Structural
designed signed by structural engineer D. Sanitary engineer for chamber 3
Plumbing engineer for instruction of piping and fitting works F. Electrical
plan for electrical engineers G. Electronics for the computers and sound
system H. Mechanical for the gen set, plumbing and all motors K. Fire
Sprinkler sign by plumbing or mechanical engineer then make a plan for
the building structure together with the specification of its discipline to be
signed & sealed with all engineers and architect after that prepare for
bidding to the lowest bidder of different aspect of the building structure if
there is a winner bidder its time or schedule to be awarded lowest bidder.
After that its time to signed all documents and blue print for preparation
for building permit. Then inspection the site where the building structure
to be located for assessment and questions and answer of inquiry then its
time to process now the building permit for the construction of the project.
It is now the time to fenc3e the perimeter of the area where the building
construction begin clearing, then start using the steel meter for the
preparation of excavation works for footing or foundation. What to prepare
in this area are comfort rooms, temp shelter or bank house for workers,

dining area, medical facilities, the fire fighterfield engineer office and
supply warehouse. Do not start the project construction without the
approved plan and drawings for all discipline by two corresponding proper
authorities shall be provided and submitted to the issuing department. It is
strictly prohibited and mortal sin to start a project structure without an
approved construction permit.

5. Impact(s) generated by the above solutions and judgements

Regarding the above solutions, early and proper planning, networking,


multimedia intervention, participation of tripartite meeting, local Gov’t
action, emb-denr required ecc, fire department and the fire brigade Filipino
& Chinese fire volunteer for their expertise in training in fire fighting
procedure system. Using the recycled waste water for additional source of
water for additional source of water for fire fighting where in digos city
have scarcity of water sources. The introduction of wastewater recycling
plant this project of one (1) unit sump pump, one unit sprayer trickling
filter, two units rotary biological contactor (rbc) complete with gear motor
and rotary disk, one unit mmf-booster pump, two unit dosing system and
one unit multi-media filter. Process description of this are one unit of sump
pump extract 300 cu.meters of waste water a day 300 cm/day from the 3
stages underground concrete receiving tank then the waste water will be
feed to the sprayer trickling filter to the sprayer trickling filter to develop
the same layer. From the trickling filter. The waste water will go to rotary
biological contractors (rbc) to facilitate the removal of contaminant and
reduced biological oxygen from rbc or rotary biological contractor, the
aerated waste water will boost to a 30’ x 72’ h multi-media filter. Sand and
carbon in the multi-media filter traps small particulate matter which are
later removed during backwash processed, filtered water from multi-media
will goes to the product tank where chlorine is dosed on the line by a
17gpd dosing pump at 250 psi to disinfect the recycled water, destroying
bacteria. Then waste water from product tank is ready for use. Start – up
sequence check electrical connection. Be sure they are all tight. Observe
the motor control panel before start up before switching “on” branches
circuit breaker (bcb), ensure that all selector switches are turned “off” so
that the system will not run automatically 2 turn “on” the main circuit
breaker (bcb). Note: before switching on the sump pump control, ensure
discharge value are open 4 switch on the selector switches at rbc 1 & 2.
Note: before switching on the booster pump of multi-media filter, ensure
rotary biological contractor (rbc) tank is full of water, please see
positioning of valves on the attached manual of multi-media filter. 5 switch
on the selector switch of dosing pumps. After switching on ensure dosing
pumps priming normally to avoid broken diaphragms. Shutdown sequence:
1. Switch off the selector of the sump pumps at the lift station and
receiving tank 2. Closed the following valves a. Booster valves b. Sump

THE PTC CODE OF ETHICS

PREAMBLE

It is a solemn responsibility of a PTC member, after admission in the practice of a


particular profession to adhere to the fundamental principles of his profession guided by
the norm of conduct consistent with this Code of Ethics.

ARTICLE 1
DECLARATION OF PRINCIPLES

Rule 1. The practice of a particular allied Technological Profession is a privilege granted by


the state. The privilege requires high degree of knowledge, efficiency,
responsibility, and morality. The dignity and honor of the profession must at all
times be kept unblemished.

Rule 2. The PTC Member shall, in the practice of his profession, be governed by the
Golden Rule, and the ideals of services to man and his environment.

Rule 3. The duly constituted authorities shall be recognized and obeyed in the practice of
his profession.

ARTICLE II
RESPONSIBILITIES TO THE STATE

Rule 1. The PTC member shall maintain allegiance to the Republic of the Philippines,
support the Constitution and obey all laws duly promulgated and approved by the
duly constitute authorities.

Rule 2. He shall strive for the common good and shall contribute his expertise for the

Rule 4. He shall avoid and discourage sensational, exaggerated and unwarranted


statements that might induce participation in unsound enterprises.

Rule 5. When he discovers faulty or unsafe devices, equipment, or machinery in any


place, work or plant, he shall without delay, give notice thereof to the proper
authority, public or private, for its immediate and effective correction.

ARTICLE IV

RESPONSIBILITIES TO EMPLOYER AND CLIENTS

Rule 1. The PTC Member must faithfully use his knowledge and skills on performing his
tasks to his clients or employer.

Rule 2. He shall protect with utmost professional attention the interests of his employer or
client.

Rule 3. He shall not advance or promote any interest of his client or employer which is
contrary to law, public or professional obligations and ethics.

Rule 4. Whenever his obligations to his employer or clients are in conflict with his
professional obligations or ethics, he should endeavour to resolve the conflict in
accordance with the law public policy, and the provision of this code. When the
conflict is beyond reconciliation, he should serve his professional engagement.

Rule 5. He shall not divulge any information given in confidence during his employment,
neither shall be attempt to profit there from nor shall he use, directly or indirectly,
such information to the prejudice of his employer and other contracting parties
shall be faithfully determined and discharged

Rule 2. He shall be willing to share the substantial profits from his enterprise with the
employees responsible thereof.

Rule 3. He must observe and obey all laws, rules and regulations on labor, particularly
those affecting labor relations, hours of work, wages and terms and conditions of
work.

Rule 4. He shall at all times strive to improve the knowledge and skills of his employees,
provide safety measures for his men, and maintain the operations efficiency of his
machines or equipment.

ARTICLE VI
RESPONSIBILITIES TO COLLEAGUES

Rule 1 ”To thine oneself be true and thou cannot be false to any man “expresses the duty
which a PTC Member owes to himself and to others.

Rule 2. He shall use only fair means to gain professional advancement. In the pursuit
thereof, he shall not injure the work employment opportunities of others, or use
any improper or questionable method of soliciting employment of clientele.

Rule 3. He shall not fraternize with those engaged in unethical practices.

Rule 4. He must duly acknowledge the professional work of his colleagues and
subordinates and give full recognition thereto.

Rule 5. He shall define clearly the line of authority and the scope of responsibility of his
associates and assistants, orders or the recall thereof, shall be issued through
proper channels.

Requirements for Registration

NOTE 1

1. The applicant should have attended and completed an accredited or


recognized engineering program by the appropriate authority in the
country.

2. Been Assessed as eligible for independent practice

2.1 The Assessment maybe undertaken by the Monitoring Committee, by


a competent Professional Association and/or by an authority with
responsibility for registration or licensing of professional engineers.

3. Gained a minimum of seven years (7) of professional experience after


registration.

3.1 The works in question should be clearly relevant to the fields of


engineering in which the applicant claims experience.

3.2 Should have participated in a range of roles and activities appropriate


to these fields of engineering

NOTE 2

1. Spent at least two (2) years in responsible charge of significant


Engineering Work.

1.1 The Work should have required the exercise of independent


engineering judgement, the project concerned should have been
substantial in duration, cost and complexity and the applicant should
be personally accountable for the success or failure of the project.

1.2 In general, an applicant may be taken to have been in responsible


charge of significant engineering work when they have:

a. Planned, designed, coordinated & executed a small project; or

b. Undertaken part of a larger project based on an understanding of


the whole project; or

c. Undertaken novel, complex and/or multi – disciplinary work.

2. Maintained their continuing professional development at a satisfactory


level. CPD to be submitted must be the latest, last 3 years.

3. Bound by the established Code of Professional Conduct or Ethics.

4. Held individually accountable for their actions as a professional engineer.
